A six-month follow-up study of 32 older people returning to the community after surviving a stroke showed considerable obstacles to successful care - management and to the operation of a 'mixed economy' of care. Genuine participation by users in arranging their own care required large adaptations in established habits of behaviour and fundamental shifts in values.
